How do I make an appointment?

  1. Choose the date/location of the clinic that works for you here.
  2. Make an account with username and password
  3. Once logged in, “Book an Appointment.”
  4. Choose the time slot.
  5. Enter all the required information.

Other details:

  • The registration system is not the same system as My Health platform; new log in creation is required.
  • The username and password you used last year for flu shots will work.
  • If you do not remember, please reset your password.
  • An auto confirmation email and on-screen confirmation will appear.
  • Be aware the auto email may enter your other folder.

Is this for Employees only?

The primary goal of these clinics is to offer convenient access to preventative health measures to protect our employees in flu season.  Employee families with BCBSVT insurance can also make an appointment if the convenience supports your family’s ability to access the vaccine.  Each person receiving a vaccine requires their own appointment. 

What type of Flu Vaccine is being administered?

The clinics do not have the vaccine suggested for 65 plus age group, please reach out to your provider or local pharmacy for supplies and appointments for this other vaccine.

Is there also a Covid Vaccine Booster?

UVM Employee Wellness is not offering any clinics with the Covid-19 boosters.  Please call your primary physician office or local pharmacy for access to Covid-19 vaccines.

The Vermont Department of Health is also a resource for Covid-19 testing & vaccines. 

How can I reschedule my appointment?

UVM Employee Wellness cannot see employee registration or help cancel or reschedule. Employees can log in (using any of the links for the clinic days):

  • Choose “APPOINTMENTS” in top right screen once logged in.
  • From the “APPOINTMENTS” page there are options:
    • request another confirmation email
    • cancel appointments
    • reschedule a new time slot for the same day
    • you cannot adjust days.  If a new day is needed, please cancel, then return to the options of days, and choose a new day and make a new appointment.

Where will my appointment be?

When you choose your time on the Employee Wellness Flu Clinics webpage, you will see the corresponding location. 

How can I get a record of my flu vaccine?

Employees can send a secure email to wellness@uvmhealth.org confirming their name and DOB. It is also incredibly helpful and makes the request much quicker if you can provide the date that the vaccine was received.

Employees will need to submit an email from their account with this information to prove that they are originating the request.

Then, UVMMC can provide a receipt for the vaccination.
